If your order includes Oxygen, CPAP masks, machines or heated humidifiers, we must have your prescription on file before we can ship your order. Please note that DME supplies and parts such as filters, tubing, water chambers, mask parts, cushions and pillows do not require a prescription.
There are three easy ways to give us your prescription:
When you fax or email a prescription, please write your order number on the prescription so we can match it to your order.
Your prescription can be handwritten on a standard prescription pad. It must include the physician’s name, contact information and signature of the care provider; your name; and a statement about the equipment needed, for example “Oxygen at LPM”, “CPAP”, “BiPAP”, “CPAP Mask”, “CPAP Humidifier” or “CPAP Supplies”. Ideally, the prescription for a CPAP machine will also include a pressure setting or oxygen will include LPM. If this information is not included on the prescription, it may be provided by the patient.
Note: Under HIPAA regulations, you have a right to request a prescription from your physician. If you have difficulty accessing your prescription, please contact our Customer Service team for assistance.

Do you have a prescription form?
Yes! AMZ Supplies’s Prescription Form may be completed and signed by your physician. You or your physician may send us your completed prescription form.
Who can write a prescription?
The prescription can be written by any of the following care providers:
- Medical Doctor (MD)
- Doctor of Osteopathy (DO)
- Psychiatrist
- Physician Assistant
- Nurse Practitioner
- Dentist
- Naturopathic Physician
We cannot accept prescriptions for CPAP equipment from the following medical professionals unless they are also one of the above:
- Chiropractor
- Podiatrist
- Optometrist
- Psychologist
Can my prescription be written in a language other than English?
Yes. We can accept prescriptions written in any language from a U.S. Physician.
Will you accept a CPAP prescription from a doctor outside the US?
No. For orders within the United States, we require a prescription from a U.S.-licensed physician. We cannot accept prescriptions from doctors outside of the United States for orders being shipped to a U.S. address.

What does a CPAP prescription need to say?
Below we’ve outlined the required elements of prescriptions for various types of CPAP equipment. All medical prescriptions must include the patient’s name, the prescribing physician’s full name, the physician’s contact information and the physician’s signature.
CPAP Machine Prescription
- One of the following phrases: “CPAP”, “CPAP Machine”, or “Continuous Positive Airway Pressure”.
- A specific pressure setting, e.g., “10 cmH2O” or simply “10”.
Auto CPAP Machine Prescription
- One of the following phrases: “APAP”, “Auto CPAP”, “Auto-set”, “Auto Adjusting CPAP”, “Self Adjusting CPAP”, “CPAP”, or “Continuous Positive Airway Pressure”.
- Optional: a pressure range, e.g., “4-20”.
- Optional: a specific pressure setting, e.g., “10”.
BiPAP Machine Prescription
- One of the following phrases: “BiPAP”, “Bi-Level”, or “VPAP”.
- Two separate pressure settings: an IPAP (Inhalation) and an EPAP (Exhalation) setting, for example: “IPAP 10, EPAP 6” or simply “10/6”.
- Optional: an inspiration time or CPAP Pressure or Expiratory Pressure Relief, for example: “IPAP 12, EPAP 8, I-Time 1.0” or simply “12/8/1.0”.
BiPAP Auto Machine Prescription
- One of the following phrases: “BiPAP”, “Bi-Level”, “VPAP”, or “BiPAP Auto”.
- A pressure range for both inhalation and exhalation, or a fixed set of pressures for both inhalation and exhalation.
BiPAP ST Machine Prescription
- One of the following phrases: “BiPAP ST”, “Bi-Level ST”, “VPAP ST”, or “Bi-Level with Backup Rate”.
- A backup rate setting, e.g., “12 BPM”.
- Optional: an inspiration time or CPAP Pressure or Expiratory Pressure Relief, for example: “IPAP 12, EPAP 8, I-Time 1.0, Backup Rate 12” or simply “12/8/1.0/12”.
- Optional: an inhalation pressure range and an exhalation pressure range, for example: “IPAP 10-15, EPAP 6-10, Backup Rate 12”.
BiPAP Auto SV Machine Prescription
- One of the following phrases: “BiPAP SV”, “VPAP SV”, or “Bi-Level Servo-Ventilator”.
- May or may not contain a backup rate or Breath Per Minute (BPM) setting.
- Contains your IPAP Min and Max and your EPAP Min and Max, or a pressure range for your IPAP and your EPAP, for example: “IPAP Min 10, IPAP Max 20, EPAP Min 6, EPAP Max 15” or simply “IPAP 10-20, EPAP 6-15”.
- Contains your pressure range for your inhalation and your exhalation, or a fixed set of pressures for both inhalation and exhalation, for example: “IPAP 10-15, EPAP 6-10” or simply “10-15/6-10”.
BiPAP AVAPS Machine Prescription
- Contains one of the following phrases: “BiPAP ST”, “VPAP ST”, “Bi-Level ST”, “Bi-Level with Backup Rate”, or “Average Volume Assured Pressure Support”.
- Contains a Tidal Volume (Vt) setting.
CPAP Mask Prescription
- One of the following phrases: “CPAP Mask”, “CPAP Interface”, “CPAP Humidifier”, “CPAP Supplies”, “CPAP”, “Continuous Positive Airway Pressure”, “BiPAP Mask”, “Bi-Level Mask”, “VPAP Mask”, “Bi-Level Humidifier”, “VPAP Humidifier”, “Bi-Level Supplies”, “VPAP Supplies”, “Bi-Level”, or “VPAP”.
